Do I need an occupational therapy assessment before having adaptations done?
Not for something you are buying and fitting yourself - anyone can put up a grab rail. An assessment becomes important when the change is structural, when it is being funded by a council or housing association, or when it is not obvious which change would actually help. Funding bodies generally want professional evidence of need before they agree to major work.
Can you arrange the building work as well?
No. An occupational therapist assesses, recommends and writes it up; the work itself is carried out by builders, fitters or the council's own contractors. Keeping those roles separate is deliberate - the person recommending the work has nothing to gain from recommending more of it.
Will adaptations make my home look institutional?
They need not. Rail finishes, seat styles and shower screens have moved on considerably, and a lot can be done with fittings that read as ordinary bathroom fixtures. It is a reasonable thing to raise at the assessment, and a reasonable thing to be given options about.
